You must have Acrobat Reader Version 7 installed on your computer in order to use computer keyboard-fillable and e-mail-submission features.

INSTRUCTIONS

  1. Open New Form I-20 Request (PDF document)
  2. Simply use your computer keyboard to fill in each blank
  3. Use your TAB key to move from blank to blank
  4. Press the Submit button when complete
  5. Carefully follow the instructions (You will have to save the file and then attach it if you use a Web-based E-mail program like Hotmail and do not have a computer-resident mail program such as Outlook™.)

If you prefer, you may obtain a New Form I-20 Request in our office, complete it there, and leave it for processing.
