    In order to maintain your lawful status and apply for non-immigrant benefits such as employment and program extensions, you will be required to complete various forms, requests and applications during your time at BYU.

    • Most forms below include a link to a computer keyboard fillable PDF file.
    • Some can be sent by E-mail to International Services while others must be printed, signed and mailed, e-mailed or brought in to International Services or sent to USCIS.
    • To access, fill in, and print the forms, you must have at least version 7.0 of Adobe Reader. If you need assistance, please contact International Services.
    • International Services forms will usually be processed in three to five working days.

    Request for Approval of Curricular Practical Training
    • Used to request approval for curricular practical training (CPT) employment
    • Submitted to International Services as part of a packet of required documents. If approved, a new I-20 is generated showing the authorization.

    On-campus Employment Verification—Contract, Student Acknowledgement of Requirements (2 pages)
    • Used to verify your lawful status in order to engage in on-campus departmental contract employment

    On-campus Employment Verification—Degree Seeking Students (2 pages)
    • Used to verify your lawful status in order to engage in on-campus hourly employment

    • Used to verify your ELC and lawful status in order to engage in on-campus hourly employment

    • Used to submit a change of address that is different than the one listed in your pending OPT application materials

    • Used to request a new Form I-20 to be included in your Form I-765 application materials sent to USCIS for extending your OPT

    • Used to request a new Form I-20 to be included in your Form I-765 application materials sent to USCIS for pre-completion OPT

    Social Security Number Application, SS-5
    • Used to apply to the Social Security Administration (SSA) for a Social Security Number (SSN)
    • Submitted to the Provo SSA office along with the SSN Verification Letter and job offer letter

    Social Security Certification Letter Request
    • Used to apply to the SSA for a Social Security Number and results in a letter that becomes part of your SSN application materials (include your department job offer in a separate e-mail or fax)
